to personal data processing In accordance with the Law “On Personal Data”, I, the person has who accepted the offer posted in the information system in the Internet (on the page at: https://ghrs-group.com/ in the sections “Rehabilitation” and “Professional Development” of the GHRS platform), and
in order to conclude and execute the Agreement on the use of the website materials and services of the (User agreement) (hereinafter referred to as the Agreement) in the User's personal account (Personal Data Subject) on the website of ghrs-group
I give my consent to the Personal Account Administrator to the processing of my personal data, namely:
- last name, first name;
- gender, age;
- date and place of birth;
- phone number (cell);
- email address;
- bank card details,
that is, to perform the actions provided in the Law “On Personal Data”, namely:
performed, using or not using automation tools, with personal data, including collection, recording, systematization, accumulation, storage, clarification (update, change), extraction, use, transfer (distribution, provision, access), depersonalization, blocking, deletion, destruction of personal data. Transfer actions (distribution, provision, access) do not apply to the User’s bank card.
Details of the Operator’s information resources, that will be used to provide any User with the access to the User’s personal data: ghrs-group.
I am informed that the Operator guarantees the processing of personal data in accordance with the current legislation of Israel, both in non-automated and automated ways.
This Consent is valid from the date of its signing until the purpose of processing personal data is achieved (until the Agreement completion or termination) or until the day of withdrawal of the Agreement on my application in writing, which can be submitted at any time.
This Consent is an integral part of the User Agreement and comes into force from the moment the User expresses consent to its terms by putting a mark of consent (tick) in a special field next to the link to the text of the Consent opposite the phrase “I have read the Consent to personal data processing”.
I confirm that by giving such Consent, I am acting of my own free will.
